Clinical presentation: a 69-year-old woman visited us because of a multinodular goiter known for more than 20 years. She was never examined because of her thyroid disorder but recently her neighbor was found harboring thyroid carcinoma which inclined our patient to initiate thyroid evaluation.

Palpation: a firm nodule in the left lobe.

Laboratory test: TSH 0.44 mIU/L.
