Clinical presentation: a 43-year-old man was referred for aspiration cytology. He had discovered a lump in the right side of the neck for several months.

Palpation: there was a firm lump lateral to the right thyroid. The right lobe was nodular on palpation.

Laboratory test: TSH 3.89 mIU/L, aTPO 0 U/mL.

Lesion in question:

the large nodular area composed of multiple discrete lesions in the right lobe.
