Clinical presentation: a 49-year-old woman has been known having Hashimoto's thyroiditis and a nodular goiter for more than 10 years. The aTPO level was 194 U/mL five years prior to the present visit. She felt that her thyroid increased in size in the last year.

Palpation: the right lobe was enlarged, firm and had an uneven surface.

Laboratory test: TSH 1.46 mIU/L.
