Clinical presentation: A 19-year-old woman was referred for evaluation of a newly diagnosed hypothyroidism. Her mother was treated for hypothyroidism and we recommended that her daughter be examined for TSH every three years. This is how the elevated TSH turned out.

Palpation: no abnormality.

Laboratory test: TSH 6.78 mIU/L.

Diagnosis. Primary hypothyroidism caused by Hashimoto's thyroiditis.

Suggestion: daily 50 microgram levothyroxine replacement therapy. TSH check in 3 months.
