The proportion of lymphocytes may be extremely low in the case of lymphocytic thyroiditis. In such cases the clear cytological distinction between a Hashimoto's thyroiditis and a Hürthle-cell tumor may be very difficult or even impossible. The presence of even a few number of lymphocytes within oxyphilic cell groups (first row of images) or the presence of nuclear debris (second row of images) and the lack of prominent nucleoli and that of the tendency of cells to dissociate (third row of images) are the clues in cytological differentiation.
