The shape of a malignant lymph node is irregular, frequently lobulated in those cases where great part of the lymph node is replaced by a tumor.

A typical, benign lymph node is presented with regular hilum, a hypoechogenic structure and lack of vascularization on Doppler mode.

Great proprtion of the node is replaced by tumor. Note the irregular surface of the node, the lack of hilum and the irregular vascularization.
