Clinical presentation: A 67-year-old woman was referred for evaluation of a nodular goiter which has been known for decades. The patient had already had difficulties in swallowing and has noted a deepening in her voice for a couple of years. She requested evaluation of the thyroid several years ago when ultrasound and scintigraphy were performed. The latter disclosed a decreased uptake in the right lobe while a normal uptake in the left lobe.

Palpation: Both lobes were enlarged and nodular on palpation.

Laboratory test: TSH 0.27 mIU/L, FT4 15.9 pM/L.

Lesion in question:

the large nodule occupying almost the entire left lobe.

Additional information on the lesion

Dimensions (width x depth x length): 59x39x73 mm.

Taller-than wide sign present: absent.

Intranodular vascularization: absent.

Pathologic adenopathy: absent.
