Clinical presentation: A 55-year-old woman has been known to have a multinodular goiter for six years. Total thyroidectomy was advised by another colleague. The patient requested a second opinion.

Palpation: a large multinodular goiter with a very firm nodule in the left lobe.

Laboratory test: TSH 4.94 mIU/L, aTPO 32 U/mL.

Lesion in question:

the nodule in the ISTHMUS.

Additional information on the lesion

Dimensions (width x depth x length): 22x17x23 mm.

Taller-than wide sign present: absent.

Intranodular vascularization: absent.

Pathologic adenopathy: absent.
