Upper-middle part of the left lobe, transverse scan. The issue is the primary cause of the protrusion and the deflection caused by the protusion. In the event of thyroiditis, the primary protrusion is very likely caused by the underlying thyroiditis (yellow arrow), and therefore the projections marked with red arrows are only secondary.
