Right lobe, transverse scan. There is a hypoechogenic nodule in the ventromedial part of the lobe. The nodule presents all three possible signs of extrathyroidal extension, the contour is abutting and bulging and the capsule is discontinuous at the relevant part. The nodule is highly suspicious infiltrating the sternocleidomastoid muscle.
