Clinical presentation: A 51-yr-old woman was referred for evaluation of a nodular goiter which has been discovered on ultrasound screening.

Palpation: no abnormality.

Laboratory tests: TSH 1.56 mIU/L.

Ultrasonography. The thyroid was echonormal. The left lobe had nodule, which had undulated margin. The lesion seemed to be moderately hypoechoic on transverse scan, while did echonormal on longitudinal view.

Cytology resulted in benign lesion.

Comment. This case illustrates the issue of interpretation regarding both the borders and the echogenicity of the nodules.

  1. Although the nodule has clearly undulated surface, it might be debated whether the degree of the irregularities reaches the pathological extent or not.
  2. The echogenicity might differ on transverse and longitudinal scan because of technical reasons. In this case, the probe was difficult to fit to the neck of the patient on transverse scan, therefore this view was the deceptive one.
