Clinical presentation: A 77-year-old woman was referred for evaluation of diffuse complaints suggesting hypothyroidism.

Palpation: no abnormality.

Functional state: euthyroidism with TSH 2.03 mIU/L, FT4 14.1 pM/L, aTPO 3 U/mL.

Ultrasonography. The thyroid was minimally hypoechogenic. There was a small cystic area in the central part of the right lobe. The maximal diameter of the lesion was 4 mm. It contained small hyperechogenic figures caused by posterior back wall enhancement.

Diagnosis: a healthy thyroid. Two of the cystic lesions were aspirated and cytology resulted in benign cystic-colloid lesion in both locations.
