Clinical data: A 46-year-old woman was referred for evaluation of a thyroid nodule which was discovered on screening.

Palpation: a not firm nodule in the right lobe.

Laboratory test: TSH 2.89 mIU/L.

Ultrasonography. The thyroid was echonormal. There was an isoechoic nodule in the upper dorsal part of the right lobe while a hypoechoic mass was found in the ventral part or ventral to the right lobe.

Cytology was performed form the hypoechoic lesion resulted in benign follicular proliferation.

Comment. In lesions which are not entirely covered by thyroid tissue, ectopic thyroid should be considered. Most of these lesions are thyroid nodules and the thyroid parenchyma is either too thin to be detected with ultrasound or has been absorbed.
