Clinical data: A 78-year-old man was referred for evaluation of an enlarged right lobe which was found on CT scan after a COVID-19 infection.

Palpation: The right lobe was enlarged and hand not firm nodules.

Laboratory tests: TSH 0.45 mIU/L.

Ultrasonography. The thyroid was echonormal and had a large nodular mass. The nodule was minimally hypoechoic or echonormal and presented cystic degeneration. The lower pole of the lobe was clearly visible while the patient swallowed.

Aspiration cytology was non diagnostic, cystic fluid only.

Suggestion: TSH and ultrasound in a year.
