First examination before surgery (first row of images):

Clinical data: A 43-year-old woman requested an evaluation of a thyroid enlargement which was noticed by the patient a month ago.

Palpation: a moderately firm, large nodule in the right lobe.

Laboratory tests: TSH 2.26 mIU/L.

Ultrasonography. A heterogeneous, dominantly echonormal occupied almost the entire right lobe. The lesion had halo and showed both perinodular and intranodular vascularity. The left lobe was intact.

Cytology resulted in oxyphilic tumor.

Right lobectomy was performed.  Histopathology: oxyphilic variant of follicular adenoma.

Second examination four weeks after surgery (second row of images):

Clinical data: The patient had no complaints.

Palpation: no abnormality.

Laboratory tests: TSH 3.99 mIU/L.

Ultrasonography. Regenerative tissue replaced the right lobe. The left lobe remained unchanged.

Suggestion: THS in 6 months.

Third examination a year after surgery (third row of images):

Clinical data: The patient had no complaints.

Palpation: no abnormality.

Laboratory tests: TSH 2.87 mIU/L.

Ultrasonography. The regenerative tissue has disappeared, normal connective tissue replaced the right lobe. The left lobe remained unchanged.

Suggestion: THS in a year, ultrasound in 5 years.

Comment. It is worth comparing the presentation of the regenerative tissue with that of the normal thyroid. The former appears after surgery and remains for several months.
