Left lobe, longitudinal scan. This is a typical and relatively specific ultrasound pattern of medullary cancer: hyperechoic, cotton-like patches without acoustic shadow are within a hypoechoic nodule. Note the lack of acoustic shadow dorsal to the hyperechogenic patches.
