Clinical presentation: A 38-year-old man requested an evaluation because of a lump which has been developed for 3 weeks in the right side of the neck.

Palpation: a large firm, painless nodule in the right lobe.

Functional state: euthyroidism (TSH 1.61 mIU/L).

Ultrasonography. The thyroid was echonormal. A large cystic nodule occupied the entire right lobe. A thin solid part could be observed in the ventral wall of the lesion.

We tried to remove the cystic content of the nodule, however we failed to aspirate any amount of fluid. Cytology resulted in benign cystic lesion.

Suggestion: right lobectomy.

Histopathology disclosed follicular adenoma.
