Clinical data. A 37-year-old woman was referred for evaluation of a nodular goiter which has been known for more than 10 years. The patient has noticed a slow, continuous increase in the size of the thyroid. Scintigraphy disclosed nodules with normal uptake.
Palpation: Both lobes were nodular on palpation.
Laboratory tests: TSH 0.37 mIU/L, FT4 1.19 ng/dL, aTPO 3 U/mL.
Ultrasonography. The thyroid was echonormal. Great proportion of the lobes were composed of discrete, mostly echonormal lesions.
Cytology of a hypoechogenic area in the left lobe resulted in benign colloid goiter.
A near total thyroidectomy was performed. Histopathology disclosed benign hyperplastic nodules and lymphocytic thyroiditis.
Comment. The ultrasound presentation resembles pseudonodular pattern of Hashimoto's thyroiditis.
