Clinical presentation: A 62-year-old man was referred for evaluation of a recurrent thyroid cyst. The cyst has been aspirated more than 5-times in the previous years but has refilled within a week after each aspiration. The patient was an athlete and the nodule caused difficulties in his performance and hindered his sport career.

Palpation: an elastic nodule in the right lobe.

Laboratory tests: TSH 1.08 mIU/L, aTPO 76 U/mL.

Ultrasonography. The right lobe was composed of a large cyst with the dimensions of 53x49x80 mm, width, depth and length, respectively. The left lobe was moderately hypoechogenic.

36 mL brown fluid was aspirated. Cytology: benign cystic lesion.

A right lobectomy was performed. Histopathology: Follicular adenoma. Hashimoto's thyroiditis in the extranodular part.

Comments. Sclerotherapy had to be considered. Nevertheless, we could ensure the restoration of life quality including his sport career only with surgical removal of the lesion.
