Clinical presentation: A 34-year-old woman was referred for evaluation of a thyroid nodule detected on screening.

Palpation: The left lobe was suspicious harboring a nodule.

Ultrasonography revealed a minimally hypoechogenic inhomogeneous thyroid. There was a more hypoechogenic area in the upper ventro-lateral part of the right lobe. The lesion corresponded to a more active focus of thyroiditis and not to a nodule in a pathological sense. There was an inhomogeneous lesion in the central part of the left lobe with not of geometrical shape.

Hashimoto's thyroiditis was disclosed on aspiration cytology from the lesion in the right lobe.

Laboratory tests: TSH 3.18 mIU/L, aTPO 348 U/mL.

Combined sonographic-cytological diagnosis was Hashimoto's thyroiditis without any pathological nodule.

Suggestion: TSH determination in a year, in the event of pregnancy at once
