Nodular goiter

Clinical presentation: A 43-year-old woman was referred for aspiration cytology of a nodule detected on follow-up of hypothyroidism. She was treated with levothyroxine for 7 years.

Palpation: No abnormality.

Laboratory tests: TSH 2.11 mIU/L, aTPO 775 U/mL on daily 75 microgram levothyroxine.

Ultrasound. The thyroid was hypoechogenic and presented fibrotic changes. There was an echonormal lesion in the central part of the right lobe having an impression on its ventral wall. The border of the lesion was sharp. The lesion presented halo sign and perinodular blood flow.

Aspiration cytology resulted in colloid goiter.

Comment. The lesion is probably a nodule in pathological sense.
