Discrete lesion or nodule in Hashimoto's thyroiditis - case 7 (1459)

Clinical presentation: A 52-year-old woman was referred for aspiration cytology of a nodule detected on neck MRI scan. Thereafter a multinodular goiter was described on ultrasound examination.

Palpation: Both lobes were firm. The presence of nodule was doubtful in the right lobe.

Laboratory tests: TSH 4.04 mIU/L, FT4 13.9 pM/L, aTPO > 1300 U/mL.

Ultrasound. The thyroid was partly echonormal partly composed of hypoechogenic discrete lesions. Neither of the latter fit to a nodule in pathological sense, they had irregular shape and puzzle-like borders and had echonormal small areas.

Aspiration cytology from the hypoechogenic lesion in the left lobe resulted in Hashimoto's thyroiditis.

Comment. The lesions are unlikely being nodules in pathological sense.
