Discrete lesion or nodule in Hashimoto's thyroiditis - case 30 (812) |

Clinical presentation: A 38-year-old man was referred for evaluation of a thyroid nodule and a suspected thyroiditis detected on ultrasound. The patient was investigated because of hair loss.
Palpation: No abnormality.
Laboratory tests: TSH 1.44 mIU/L, aTPO-level 395 U/mL.
Ultrasound. The thyroid presented the so-called honey-combing pattern. The echogenicity index was around 30% in the right lobe and 20% in the left lobe. There was a minimally hypoechogenic-echonormal lesion in the upper dorsal part of the right lobe. The lesion presented halo and perilesional blood flow.
Aspiration cytology of the lesion in question resulted in Hashimoto's thyroiditis.
Comment. The echonormal lesion is probably not a nodule in pathological sense while the hypoechogenic areas are unlikely true nodules.
