Clinical presentation: A 39-year-old woman was referred for aspiration cytology. She was investigated because of 'lump in the throat' feeling.

Palpation: no abnormality.

Functional state: euthyroidism (TSH 1.08 mIU/L, anti-TPO 2 U/mL).

Ultrasonography. The thyroid was minimally hypoechogenic and inhomogeneous. There was a moderately hypoechogenic lesion in the ventromedial part of the left lobe. The lesion presented microcalcifications.

Cytology resulted in papillary carcinoma.

Histopathology disclosed papillary carcinoma and chronic lymphocytic thyroiditis.
