Clinical presentation: A 76-year-old woman was referred for aspiration cytology of a nodular goiter, which was detected on evaluation performed because of malignant melanoma. The latter was diagnosed in the left upper arm 4 months ago. Histopathology disclosed nodular form.
Palpation: The left lobe and the isthmus were enlarged and hard on palpation. Enlarged lymph nodes were also present in the left side of the neck.
Functional state: euthyroidism with TSH-level 2.83 mIU/L.
Ultrasonography. There was a hypoechogenic nodule in the left lobe and an enlarged, irregular lymph node in the left side of the neck.
Cytology was performed from the enlarged lymph node and from the nodule in the thyroid. Thick black fluid was aspirated. Cytology resulted in both cases in malignant melanoma.
Comment. The tumor presents lobulated margins.
