Clinical presentation: a 59-year-old woman was operated with follicular adenoma 2 years ago. She was referred for an evaluation of a newly detected hard nodule.

Palpation: a hard, painless nodule in the right thyroid.

Functional state: euthyroidism (TSH-level 2.74 mIU/L).

Ultrasonography: the left thyroid was intact. There was a moderately hypoechogenic nodule with hyperechogenic granules in the resected right thyroid.

Cytological diagnosis: benign, granulation around surgical thread.

The patient wished to be operated because the granulation caused aesthetic problem.

Histopathology: granulation around surgical thread.
