Clinical data: a 68-year-old woman was referred for evaluation of a nodule detected on carotid Doppler examination.

Palpation: a not firm nodule in the right lobe.

Functional state: euthyroidism with TSH 1.79 mIU/L.

Ultrasound revealed a hypoechogenic nodule in the right lobe. The lesion presented halo, taller-than wide sign and had a perinodular blood flow.

Aspiration cytology resulted in the follicular tumor.

Histopathology disclosed microfollicular adenoma.

Comment. There were atypical, pleomorphic cells on the smear. Therefore the risk of follicular carcinoma was higher than the average.
