Clinical presentation: A 65-year-old woman was referred for evaluation of low TSH detected on yearly check of hypothyroidism. She had no complaints.

Palpation. Both lobes were firm. No nodule was palpable.

Laboratory test: TSH 0.05 mIU/L, FT4 19.2 pM/L on daily 100 microrgram levothyroxine.

Ultrasonography. The thyroid was moderately hypoechogenic and had several more hypoechogenic areas. There were no nodules of pathological sense.

Suggestion: to decrease the dose of levothyroxine to daily 75 microgram.
