Clinical data: a 58-year-old woman was referred for an evaluation of a thyroid nodule and an enlarged lymph node in the right side of the neck. She was examined because of a mass in the right upper lobe of the lung detected on screening.

Functional state: euthyroidism with TSH-level 2.19 mIU/L.

Ultrasonography: an extremely large hypoechogenic nodule in the left thyroid with irregular borders.

Cytological diagnosis: anaplastic adenocarcinoma. The wash-out thyroglobulin determination of the remnant resulted in undetectable level.

Combined diagnosis: secondary cancer of the thyroid.

The patient underwent perthoracal biopsy of the mass in the lung.

The biopsy of the lung: poorly differentiated adenocarcinoma.
