Clinical presentation: a 50-year-old woman was referred for evaluation of a multinodular goiter dicovered on screening.

Palpation: a moderately firm nodule in the right lobe.

Functional state: euthyroidism (TSH 3.00 mIU/L).

Ultrasonography. The thyroid was minimally hypoechogenic. There were two hyperechogenic nodules in both lobes. The largest one in right thyroid presented cystic degenaration.

Cytology resulted in benign cystic-colloid goiter.

An anti-TPO test was done which resulted in 79 U/mL.

Final diagnosis: Hashimoto's thyroiditis in euthyroid state. Multinodular goiter. Yearly TSH determination and a repeat ultrasound in three years was suggested.
