Clinical data: A 70-year-old woman was referred for evaluation of a nodular goiter which was diagnosed during evaluation of suspected lung cancer. CT detected a mass in the left upper mediastinum.

Palpation: The left lobe had a large, not firm nodule.

Hormonal evaluation indicated euthyroidism (TSH 3.46 mIU/L).

Ultrasonography. The thyroid was echonormal. There were several insignificant lesions in the right lobe. The left lobe presented a large nodule composed of discrete hyperechogenic areas. The lower pole of the lobe could be clearly visualized while the patient swallowed (see video).

Cytology of resulted in benign colloid goiter.

Suggestion: ultrasound in a year.
