Clinical presentation. A 62-year old man requested a second opinion. A multinodular goiter was discovered on carotid Doppler examination by chance. Aspiration cytology resulted first in Bethesda 3 (atypia of unknown significance), thereafter four months later in Bethesda 5 (suspicion of thyroid carcinoma not otherwise specified) in another institute.

Palpation: no abnormality.

Laboratory test: TSH 1.39 mIU/L, FT4 10.8 pM/L, aTPO 0 U/mL.

Lesion in question:

the small lesion in the central part the right lobe.

Additional information on the lesion

Dimensions (width x depth x length): 7x8x6 mm.

Taller-than wide sign present: present.

Intranodular vascularization: absent.

Pathologic adenopathy: absent.
