Clinical presentation: A 41-year-old woman came to a follow-up. We met her three times in the past 13 years. At the first time, we aspirated 6 mL brown fluid from the nodule in the isthmus. The cyst has recurred 3 years later when we aspirated 6.4 mL fluid from the nodule. Recently, she noticed recurrence of the nodule but she experienced no symptoms other than aesthetic problem.

Palpation: an elastic nodule in the isthmus.

Laboratory test: TSH 2.01 mIU/l.

Seven mL yellow fluid was aspirated from the isthmic nodule. Cytology resulted in non-diagnostic, cystic fluid only category.

Diagnosis. Euthyroidism. Benign multinodular goiter.

Suggestion: ultrasound in two years, sooner in case of complaint. If the cyst would relapse within 5 years, then ethanol sclerotherapy.
