Clinical presentation: A 76-year-old woman came to a follow-up visit. She has been treated for hypothyroidism for more than 25 years.

Palpation: no abnormality.

Laboratory test: TSH 3.06 mIU/L on daily 50 microgram levothyroxine.

Diagnosis. Hashimoto's thyroiditis. Adequate replacement therapy for hypothyroidism.

Suggestions: continuation of treatment with a dose of 50 microgram levothyroxine per day. TSH check in a year.
