Clinical presentation: A 57-year-old woman was referred for evaluation of complaints suggesting hyperthyroidism. A total thyroidectomy was performed 11 years ago. Histopathology disclosed benign hyperplastic nodules and chronic lymphocytic thyroiditis.

Palpation: no abnormality.

Laboratory test: TSH 0.12 mIU/L, FT4 19.8 pM/L on daily 125 microgram levothyroxine.

Diagnosis. Normal postoperative ultrasound pattern. Slightly overdosed replacement therapy for hypothyroidism.

Suggestions: daily 100 microgram levothyroxine replacement therapy. TSH check in 6 months.
