Clinical presentation: A 48-year-old woman was referred for evaluation of a newly diagnosed hypothyroidism.

Palpation: no abnormality.

Laboratory test: TSH 12.6 mIU/L, FT4 9,75 pM/L, aTPO 600 U/mL.

Diagnosis. Hashimoto's thyroiditis without any nodules of clinical or oncological significance. Primary hypothyroidism.

Suggestion: daily 50 microgram levothyroxine replacement therapy. TSH check in 3 months.
