Clinical presentation: On evaluation of weight loss, a multinodular goiter was found in an 85-year-old woman.
Palpation: a firm nodule in the left lobe.
Functional state: euthyroidism (TSH 0.97 mIU/L).
Ultrasonography. The thyroid was echonormal and beside has small hypoechogenic areas, had two nodules. Both were hypoechoic. The lesion in the left lobe presented different intranodular hyperechogenic figures including macrocalcifications and comet-tail artifacts. Some granules were probably microcalcifications.
Aspiration cytology was performed from both nodules and resulted in benign colloid goiter.
Suggestion: ultrasound in two years.
Comment. It is worth analyzing the presentation of the different echogenic figures in the video. Moreover, three different settings can be compared in projecting the nodule in the left lobe.
