A 38-yr-old woman requested a second opinion before a planned surgery. She was diagnosed with a right eye melanoma. Local therapy was given. On PET CT scan non-specific positivity was found in the right thyroid lobe. Aspiration cytology was performed in another hospital and it raised the suspicion of metastatic malignant melanoma based on the color of the aspirated fluid, described as 'dark brown' and on the HMB45-positivity of macrophages.
Palpation: an elastic nodule in the right lobe.
Laboratory tests: TSH 1.47 mIU/L, FT4 17.5 pM/L, aTPO 11 U/mL.
