Clinical presentation: A 38-yr-old man was referred for evaluation of a thyroid nodule detected on evaluation of elevated blood pressure.

Palpation: no abnormality.

Laboratory tests: TSH 1.08 mIU/L, FT4 12.9 pM/L.

Ultrasonography. The thyroid was echonormal. There was a cystic lesion in the right lobe with minimal if any solid portions.

We tried to aspirate the cyst, but we gained only minimal amount of thick, pale yellow material. Cytology resulted in benign cystic lesion.

Comment. The issue is whether tiny portions at the cyst' wall should be interpreted as solid areas. If yes than the nodule belongs to peripheral-type cysts, if not than this is a pure cystic nodule.
