The echogenicity of the nodule - case 2120

Clinical presentation: A 57-yr-old man was diagnosed with a nodular goiter on evaluation performed before a cardiac surgery.

Palpation: a not firm nodule in the right lobe.

Laboratory test: TSH 1.23 mIU/L.

Ultrasonography. The thyroid was echonormal. There were several discrete lesions in the right lobe, including an echonormal one in the ventral part The dorsal nodule presented irregularities on the borders.

Cytology was performed from the hypoechoic nodule and resulted in benign lesion.

Comment.

  1. Regarding the nodule borders, the heterogeneous nodule presented lobulated margins.
  2. The dorsal nodule is either homogeneously hypoechoic (i.e. composed of deeply and minimally hypoechoic parts) or heterogeneous (i.e. composed of hypoechoic and echonormal parts) depending on the judgement of the less dark portion of the nodule.
