Clinical presentation: A 29-yr-old woman was referred for evaluation of a nodular goiter discovered on screening.

Palpation: no abnormality.

Laboratory test: TSH 1.74 mIU/L.

Ultrasonography. The thyroid was echonormal. There was a dominantly hypoechoic nodule in the left lobe. The lesion showed taller-than-wide sign and presented perinodular blood flow. Although the borders between the hypoechoic and echonormal parts were spiculated, this is not the presentation of a pathological, irregular borders. As a rule, the borders refer to the outer surface of a nodule.

Cytology resulted in follicular tumor.

A left lobectomy was performed. Histopathology disclosed a follicular adenoma.
