First examination (first row of images):

Clinical presentation: A 41-yr-old woman was referred for evaluation of a nodular goiter which has been discovered for 18 months.

Palpation: a not firm nodule in the right lobe.

Laboratory tests: TSH 1.51 mIU/L, FT4 15.8 pM/L.

Ultrasonography. The thyroid was echonormal. The right lobe had a large, dominantly echonormal nodule. The lesion showed perinodular blood flow.

Cytology resulted in benign lesion.

Suggestion: ultrasound in a year.

Second examination (second row of images):

Clinical presentation: The patient came to follow-up examination. She had no complaints.

Palpation: a not firm nodule in the right lobe.

Laboratory tests: TSH 0.74 mIU/L, FT4 14.7 pM/L.

Ultrasonography. The pattern remained unchanged as did the size of the nodule.

Suggestion: ultrasound in two to three years.

Comment. Although the borders between the hypoechoic and echonormal parts were spiculated, this is not the presentation of a pathological, irregular borders. As a rule, the borders refer to the outer surface of a nodule.
