Clinical presentation: A 42-year-old woman was referred for evaluation thyroid nodule. She was investigated because of weight gain and fatigue.

Palpation: no abnormality.

Functional state: euthyroidism (TSH 1.64 mIU/L).

Ultrasonography. The thyroid was echonormal. There was an echonormal nodule which presented halo sign and perinodular blood flow in the central part of the left lobe. Next to the dorsal part of the nodule, a minimally hypoechogenic lesion was found which contained microcalcifications and irregular borders.

Cytology was performed from the hypoechogenic lesion and resulted in papillary carcinoma.

Histopathology disclosed papillary carcinoma in the left lobe with a maximal diameter of 4 mm. The tumor gave metastases to the regional lymph node.
