Clinical presentation: A 68-year-old woman was referred for evaluation of fatigue, weight gain and 'lump in the throat' feeling.

Palpation: no abnormality.

Laboratory test: TSH 5.31 mIU/L, aTPO 54 U/mL.

Cytology resulted in benign cystic-colloid goiter.

Diagnosis. Subclinical hypothyroidism. Benign nodular goiter.

Suggestion: TSH in 6 months, ultrasound in two years.
