Clinical presentation: A 71-year-old woman came to a routine follow-up. She has been operated for 19 years with relapsing hyperthyroidism caused by Graves' disease.

Palpation: no abnormality.

Laboratory test: TSH 2.21 mIU/L on daily 75 microgram levothyroxine.

Diagnosis. Normal postoperative ultrasound pattern. Adequate dose of replacement therapy for hypothyroidism.

Suggestion: continuation of treatment with a dose of 75 microgram levothyroxine per day. TSH check in a year.
