Clinical presentation: A 64-year-old woman was referred for aspiration cytology of a nodule. On evaluation of hoarse voice which has been noticed for a couple of months, a multinodular goiter was detected. Laryngology did not reveal any abnormalities.

Palpation: Both lobes were firm and nodular on palpation.

Laboratory tests: TSH 2.04 mIU/L, aTPO 90 U/mL.

Ultrasound. The thyroid was moderately hypoechogenic and was composed of multiple hypoechogenic, echonormal and hyperechogenic discrete lesions. The pattern corresponded to pseudonodular form of Hashimoto's thyroiditis.

Aspiration cytology from a moderately hypoechogenic lesion in the lateral part of the lobe resulted in lymphocytic thyroiditis.

Comment. All but one lesions are probably not nodules in pathological sense. The classification of the moderately hypoechogenic lesion in the lateral part of the left lobe is doubtful.
