Lymphocytic thyroiditis may appear as a large nodule-like hypoechogenic area surrounded with a thin echonormal rim. This presentation mimics that of a large hypoechogenic nodule. The presence of intralesional fibrotic changes and the irregular, puzzle-like borders between the echonormal and hypoechogenic parts are the most important signs favouring lymphocytic thyroiditis.
