There is one hypoechogenic lesion in each lobe. The shape of the right one is iiregular, while the left lesion is more regular. The presence of fibrotic changes decreases the possibility that these are nodules in a pathological sense. The fact that this patient had hypothyroidism increases the possibility being these areas only signs of thyroiditis.

To summarize: we cannot decide whether these lesions are nodules or not nodules on ultrasound. If we take the underlying hypothyroidism into consideration, it will seem more likely that these are not nodules in pathological sense. Nevertheless FNAC is required. This patient underwent surgery, the final histopathology disclosed Hashimoto's thyroiditis, and there wasn't any nodule in the thyroid.
