The most important keys to the diagnosis of de Quervain's thyroiditis are the patchy hypoechogenic areas with blurred borders and the decreased vascularity. The former is demonstrated in the images in the first three rows, while the latter in the last two rows. The hypoechogenic areas with blurred borders are specific and sensitive signs, while the decreased vascularity has a less diagnostic value.
