The main difference between the two cases is that the metastatic cancer displays pleomorphic cells but papillary carcinoma does not. Nevertheless, a less well-differentiated thyroid carcinoma cannot be excluded on the cytological pattern of the left case. The determination of wash-out thyroglobulin level was zero in the metastatic carcinoma case.
